To register at the practice, please contact 02891 878391 and ask to speak to reception staff.

All individuals wishing to join the practice will be required to complete the appropriate registration forms, provide their Health and Care number (if known), photographic ID and proof of their current address. Please provide copies of these. It will also be necessary to provide proof of any current medication.

You must reside within 5 miles of the practice. 

BSO Information on Registering with a GP in NI

There are 2 registration forms for different circumstances:

1. Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200.
2. Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.

Please also complete the New Patient Registration Form below and bring this with you to reception along with all other required documentation.
