The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team, it is necessary that medical information about you is shared between members of the practice team and district nurses, hospitals or social workers in order to provide further medical care, with your consent.
If you do not wish for anonymised information to be shared with the Health Board, please let us know.
The practice complies with the Freedom of Information and Access to Medical Records Acts. Patient's may request to see their medical records, and such requests should be made via the Practice Manager.
